DESCRIPTION OF POSITION:

Rehabilitation Coach: The purpose of this position is to provide an increased level of support and assistance to foster families and foster children with significant behavioral, and/or emotional needs. MHRC will be provided where the stability of placement would be at risk without additional services and assessed as medically necessary. The services of the Coach will be based on an individualized case plan developed for each child/youth by the clinician. Specific responsibilities and duties will vary depending on the case/clinical plan, needs of child, and needs of foster parents. Rehab Coach: will provide additional services to the child/youth not receiving IHBS and identified in their plan as benefiting with assistance in improving, maintaining, or restoring their functional skills; including daily living skills, social and leisure skills and hygiene skills.

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:

  • Providing some services evenings and weekends will be required.
  • To work together with the Parent Partner and will provide non- traditional therapeutic services to help the child/youth achieve their goals and objectives of their plan
  • Develop and engage in activities with foster child to provide treatment level interventions in accordance with the client plan and child’s diagnostic needs.
  • Assist foster parents in implementing behavioral and skill building objectives to enhance the goals set forth in the client plan. Plans will include designing and implementing reinforcement systems to motivate appropriate behaviors, and teaching and modeling functional skills, such as self-help skills and behavioral redirection.
  • Intervene in crisis prevention and intervention.
  • Actively participate as member of the Child and Family Treatment Team; assist in developing goals and objectives for each child.
  • Responsible for documentation as set forth by the requirements for the EPSDT program documentation requirements for this position.
  • At all times project a favorable image of Walden Family Services.
  • Perform other duties as assigned or requested.

Travel is required for this position.

Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.

Requirements:

QUALIFICATIONS REQUIREMENTS:

  1. Must have Bachelor's Degree.
  2. 2 years of experience working with youth-at-risk or dually–diagnosed children/ youth in residential, community, or school settings.
  3. Knowledge in behavioral management techniques and behavioral plan implementation
  4. Able to establish effective rapport with children, adolescents, foster and biological families.
  5. Must possess valid California Driver’s License and have a good driving record.
  6. Fingerprint and Child Abuse Index clearances from the Department of Justice.
  7. Must meet all requirements set by Community Care Licensing pertaining to the hiring of employees by a licensed community care facility.

EMPLOYEE STATUS: Non-Exempt

SALARY RANGE: $22.00 - $24.00 per hour
