Key Responsibilities: Patient Assessment: Conduct comprehensive initial and ongoing assessments of patients' health status, including physical, emotional, and psychosocial needs.
Care Planning: Develop individualized care plans in collaboration with patients, their families, and the interdisciplinary healthcare team, addressing specific needs and goals.
Coordination of Care: Coordinate and facilitate the provision of healthcare services, therapies, and medical equipment as prescribed in the care plan.
Patient Education: Provide patients and their families with education on their medical condition, treatment options, and self-care techniques to enhance their independence and well-being.
Medication Management: Administer medications and monitor medication adherence, side effects, and interactions, ensuring patient safety.
Clinical Documentation: Maintain accurate and up-to-date patient records, documenting all assessments, interventions, and changes in the patient's condition in compliance with regulatory standards.
Monitoring and Evaluation: Regularly evaluate the patient's progress toward their goals and make necessary adjustments to the care plan as needed.
Advocacy: Serve as an advocate for patients and their families, ensuring their needs and preferences are effectively communicated and met within the healthcare system.
Team Collaboration: Collaborate with other healthcare professionals for the benefit of your patient.
Quality Assurance: Participate in quality improvement initiatives to enhance the delivery of care and maintain compliance with regulatory standards.
Emergency Response: Be prepared to handle medical emergencies, assess the situation, and take appropriate actions to stabilize the patient.
Qualifications: - Current RN licensure in state of Indiana
- Current CPR certification required
- Current Drivers License, vehicle insurance, and access to a dependable vehicle
- Strong assessment, care planning, and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a collaborative healthcare team.
- Strong computer skills for documentation and communication purposes.
Physical Requirements: - Ability to lift and move patients and equipment as needed.
- Comfortable with extended periods of walking, standing, and bending.
- Adequate vision, hearing, and manual dexterity.
