Come Grow with Us! HOPE Community Services is partnering with public schools in South Oklahoma City to provide quality school based services. We are looking for an individual that enjoys working with children and has a passion for helping others create positive changes. 

 

SUMMARY

Provides support to Systems of Care clients and their families by assisting with day-to-day activities in the home, school and other community settings to enable the family to remain intact and the student to remain in school. 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. 

 

  • Adheres to agency policy and procedures, as found in the Forms Locked drive on each computer. Also adheres to CARF and DMHSAS standards and criteria, federal and state confidentiality rules and regulations, and any special funding source requirements including Medicaid and HCA regulations. 
  • Establishes and maintains professional working relationships with supervisor and peers. Can effectively work within a team or group setting cohesively. 
  • Responsible for maintaining strictest confidentiality guidelines for interactions regarding consumer and family information.
  • Develops and maintains positive relationships with community partners. Participates in multidisciplinary team, school-related meetings, and educational activities.
  • Assists client with various day-to-day activities including maintaining System of Care/BISS Clients in school/classroom and home environment. Assists with client and family needs and activities.
  • Contributes observations to staff for Wrap planning, Family Team Meetings, faculty and parents to enhance the student’s academic and behavioral success.
  • Completes paperwork to document activities and observations in accordance with agency policy and procedures to be completed no later than the next business day. Utilizes the philosophy of Collaborative/Therapeutic Documentation to meet clinical record keeping documentation standards and ensure documentation is completed with the Consumer to the extent possible and all documentation is completed on the same day of the date of service.
  • Collects any data as directed for evaluations and research of program effectiveness. 
  • Participates in group outings as needed and assures safety for clients and their family.
  • Participates in ongoing in-service education programs and staff development programs.
  • Supports and promotes a service environment for individuals in need of behavioral health and/or co-occurring services that is recovery focused, welcoming, and attentive to the needs of individuals who may have experienced trauma in their lives. Attends annual core competencies training in relationship to individuals with co-occurring disorders or who have experienced trauma in their lives.
  • Attends all required staff meetings, workshops, and seminars to learn Agency policy, rules, regulations, and procedures as well as to meet any needed agency requirements.
  • Maintains working knowledge of current trends and developments in the mental health field by through appropriate reading, attendance of conferences and workshops, and consultation with other professionals.
  • May be required to work evening or weekend hours and adopt a flexible work schedule to accommodate the needs of students and school partners. All work schedules should be communicated and approved by the Director in advance.
  • Travel, using personal vehicle, will be required. May be required to transport consumers in personal vehicle.
  • Performs other related duties as assigned.

EDUCATION and/or EXPERIENCE

High School Diploma or equivalent, 

6 months of Behavioral Health Experience

 

CERTIFICATES, LICENSES, REGISTRATIONS

In order to provide client transportation in private or agency vehicles, must possess current, valid Oklahoma license, and meet HOPE's vehicle insurance requirements. HOPE will annually obtain copy of Motor Vehicle and Record from the State Department of Public Safety. 

 

Obtain and maintain BHA Certification through ODMHSAS. 

Obtain and maintain BHCM I Certification through ODMHSAS

Required to complete Therapeutic Options training and annual updates.
